Privacy Policy

This Privacy Policy describes how advancedprimarycaremanagement.com (the “Site”) collects, uses, and protects information from visitors. The Site is published as an editorial resource for primary-care practices and value-based-care professionals. It does not collect protected health information (PHI) and is not intended for use by patients or Medicare beneficiaries.

Information we collect

Information you submit through forms. When you submit the “See if APCM fits your practice” form or any other form on the Site, we collect the information you provide — typically your name, practice name, role, email address, and any free-text context you choose to share. This information is transmitted to the publisher’s editorial and partnership team to respond to your inquiry.

Automatically collected technical information. Like most websites, the Site logs standard technical information when you visit: IP address, browser type, referring URL, pages viewed, and timestamps. This information is used for security, troubleshooting, and aggregate analytics.

Analytics. The Site may use privacy-preserving analytics to understand which content is read, how visitors arrive, and where they exit. Analytics data is aggregated and does not include personally identifying information.

Cookies. The Site uses a minimal set of cookies necessary for site function. We do not use third-party advertising cookies or cross-site tracking cookies.

How we use information

Information submitted through forms is used to respond to your inquiry, connect you with the editorial team or implementation partners as appropriate, and follow up on your request. Technical and analytics information is used to operate, secure, and improve the Site.

We do not sell information you submit. We do not share information you submit with third parties except (a) implementation partners you have asked us to connect you with, (b) service providers that operate Site infrastructure under confidentiality obligations, and (c) where required by law.

No PHI collected

The Site is intended for primary-care practice operators, billing and revenue-cycle teams, value-based-care strategy professionals, and adjacent audiences. The Site does not request and should not be used to submit protected health information about specific patients. If you are a patient seeking medical care, please contact your primary-care provider directly.

Data retention

Form submissions are retained for as long as reasonably necessary to respond to your inquiry and maintain a record of the interaction. Technical logs are retained for a limited operational window. You may request deletion of your information by contacting the publisher.

Your choices

You may unsubscribe from any follow-up communications at any time using the unsubscribe link in those communications or by contacting the publisher directly. You may also request access to, correction of, or deletion of the information you have submitted.

Security

We use commercially reasonable administrative, technical, and physical safeguards to protect the information collected through the Site. No internet transmission is fully secure; we cannot guarantee absolute security.

Changes to this policy

We may update this policy from time to time. Material changes will be reflected in the “Effective” date at the top of this page.
