This Privacy Policy describes how advancedprimarycaremanagement.com (the “Site”) collects, uses, and protects information from visitors. The Site is published as an editorial resource for primary-care practices and value-based-care professionals. It does not collect protected health information (PHI) and is not intended for use by patients or Medicare beneficiaries.

When you submit the “See if APCM fits your practice” form or any other form on the Site, we collect the information you provide — typically your name, practice name, role, email address, and any free-text context you choose to share. Form submissions are retained for as long as reasonably necessary to respond to your inquiry and maintain a record of the interaction. Technical logs are retained for a limited operational window. You may request deletion of your information by contacting the publisher.
